Conference Web site located at www.ihcleaders.com, and all associated sites
linked to www.ihcleaders.com by Independence Holding Company, its subsidiaries
and affiliates, including IHC sites around the world (collectively, the
"Site"). The Site is the property of Independence Holding Company ("IHC") and
